Two fields. The website is the one thing we need, because it is how we know which link on a page belongs to them.
Why we ask for it
On every page you give us, we look for the link that points at this domain. That is how we know which of the dozens of links on a page is yours.
You never type the wording of a link. We read the page, find your link, and record the exact wording, where it points, and whether Google is counting it. That first reading becomes the baseline, and every later check gets compared against it.
Recording it ourselves rather than taking your word for it is the point. Nobody can hand us a list saying the wording was something it was never set to.
